If this is your first appointment at the Headache Care Center, medical forms will be mailed to you prior to your appointment. This saves time for you and allows you to jot down any questions you might wish to discuss with the doctor.

First time appointments usually take one hour. This allows time for the physician to assess your history/presenting condition, conduct the exam, discuss with you your treatment plan, and allows time for any questions you might have.

Please bring your insurance card(s) to each visit. We also need as much prior health history on you as possible at your first visit, meaning test results, records from other physicians and medications you are taking (current and past) to give you the best healthcare experience possible.

If you are unable to keep your appointment on the scheduled date, please contact us in advance so that we can reschedule your appointment. You will receive a reminder call 24 hours prior to your scheduled appointment.
