Your care begins with your first phone call to Headache Care Center. You will be greeted by our receptionist, who will ask questions about your headaches. This will assist with scheduling you into the appropriate treatment program. If you have health insurance, you will be asked to provide specific information located on your insurance card. Please have it ready when you call.
Once your appointment is scheduled, a packet of information will be mailed to you. It is important to complete all forms and bring them with you to your appointment.  The receptionist, billing coordinator, nurses, and physicians rely on the information provided on these forms.  If you do not have the forms completed on your arrival, you will be asked to complete the forms before the physician can see you, which may cause a delay in your appointment time.
On your arrival at Headache Care Center, the receptionist will make a copy of your insurance cards and driver’s license for your file. Please bring these items to your appointment.  If you are unable to present an insurance card, you will be expected to pay in full for any services received on the day of your appointment.
